This is the standard black edition of the 30th-anniversary reissue: the full performance cut across four sides of two LPs, issued by Legacy Recordings under catalog number 19802848671. The album first appeared on July 30, 1996, and until this reissue it had never been pressed on vinyl in the United States.

The tapes come from one night, April 10, 1996, at the Majestic Theater inside the Brooklyn Academy of Music in New York. It was the band's first performance in two and a half years, and the only one they played as a five-piece: Scott Olson, of Heart, joined Layne Staley, Jerry Cantrell, Mike Inez and Sean Kinney on second acoustic guitar. Toby Wright produced the album with the band.

Columbia released it that summer and it entered the Billboard 200 at No. 3. The RIAA certified the album two times platinum in 2022, and the home video that followed it is certified gold.
