Columbia has kept this catalogue number, CK 57628, in print on the same barcode since 1994. It is the original master on a single disc, in a jewel case with a ten-panel foldout booklet, and it is not the 2024 anniversary reissue.

The band wrote and cut the seven songs at London Bridge Studio in Seattle between 7 and 14 September 1993, producing themselves, then mixed at Scream Studios the following week and mastered at Precision Mastering. It was Mike Inez's first studio recording with the group.

Jerry Cantrell sings lead on "No Excuses" and "Don't Follow"; Layne Staley takes the rest, with Sean Kinney on drums. Three songs went to radio as singles — "No Excuses," "I Stay Away" and "Don't Follow."

Columbia released it on 25 January 1994 and it entered the Billboard 200 at No. 1 on more than 141,000 copies in a week, the first EP ever to do so. The RIAA certified it four times platinum in 2022.
