Back On Black's picture-disc pressing of Cannibal Corpse's third album, licensed from Metal Blade and carrying the original uncensored Vince Locke cover. The record comes in a clear plastic sleeve with a four-panel booklet of lyrics.

The band recorded the album between June 1 and 15, 1992 at Morrisound Recording in Tampa, with Scott Burns producing, engineering and mixing. The lineup was Chris Barnes on vocals, Bob Rusay and Jack Owen on guitars, Alex Webster on bass and Paul Mazurkiewicz on drums. It followed Eaten Back to Life and Butchered at Birth.

Metal Blade's Brian Slagel sent Locke's first version of the sleeve back as insufficiently graphic; the artist added the lacerations and the blue-grey skin tones that appear on the final painting. The album has had a complicated life since: it was indexed in Germany in November 1995, restricting its sale to adults, and from October 1996 every Cannibal Corpse record was banned outright in Australia, where the catalogue was reclassified for sale to over-18s in 2006 and 2007. It reached No. 182 on the Australian albums chart in 1995.

The opening track is also the band's most widely heard: "Hammer Smashed Face" appears in Ace Ventura: Pet Detective, with the group performing on screen at Jim Carrey's request. Paul Mazurkiewicz has said fans still name the film as how they found the band.

Metal Blade first issued the album on September 22, 1992.
