The cover is a saturated pink press photograph of the band in forest undergrowth, shot by Vickie Berndt while experimenting with color infrared film. A printed 12-inch insert reproduces the handwritten lyric collage. Eric Erlandson, the band's founding guitarist, oversaw the new vinyl remaster, and the packaging restores the original artwork.

Caroline Records issued Pretty On The Inside in the US on September 17, 1991, with Berlin's City Slang handling Europe, where the first 3,000 LPs were also pressed on colored vinyl, in blue. The record was made in a week at Music Box Studios in Hollywood with Courtney Love, Eric Erlandson, Jill Emery and Caroline Rue. NME gave it 9 out of 10, and Alternative Press later placed it at No. 74 in its list of the top 99 albums of 1985 to 1995.

The cover lettering was designed by Long Beach artist Pizz, and the back cover reproduces a painting by Emery.
